It shall be your own responsibility to ensure that any products, services, or information available through this website or discussed before, after or during your Treatment meet your specific requirements.

I acknowledge that I am purchasing a tooth whitening kit treatment that is designed to whiten the colour of my teeth using peroxide gel and led lamp and understand the cosmetic process of bleaching teeth will not change enamel but improve staining which whitens teeth colour. I give permission for a technician at Lit Smiles Perth to apply this gel and use the lamp on me during procedure. I understand everyone’s teeth are different and results vary and cannot be guaranteed. I understand yellowish teeth show better results and a touch up appointment is required to get better results. If I have fillings, caps, crowns, veneers, dentures any non-enamel surface will not whiten in colour with peroxide gel and led lamp. I understand potential risks may include but are not limited to gum/lip irritation, tooth and gum sensitivity, potential spots, or streaks on teeth. I understand after treatment it is normal for teeth colour to regress over time.

ELIGIBILITY: I understand that this treatment CANNOT be used on people under the age of 16, people with skin conditions that are at risk of any nonspecific jells, creams and with allergies that may be affected by any/all Treatments. If I feel a pain on treated area, I should stop the treatment and seek medical attention if needed. The treatment is not recommended for persons who have had lip filler or cosmetic tattoo within two weeks prior to the treatment.

I accept I am having this treatment at my own risk and understand the eligibility and give permission for a Technician at Lit Smiles Perth to perform my purchased tooth whitening kit and will not hold Lit Smiles Perth, its owners, suppliers, or any employees liable for any of the above risks mentioned or outcome from tooth whitening kit treatment.

Side effects may include but are not limited to, teeth sensitivity, headache, sensitive lips, swollen lips, marks on lips form check retractors and any other side effects or sensitivity from Teeth Whitening Treatment I will not hold Lit Smiles Perth for.

I have read and fully understand these Terms & Conditions including the possible risks, complications and benefits that can result from this Treatments, and that I will not hold Lit Smiles Perth, its owners, suppliers, or any of its employees liable for any risks, outcomes, side effects during and post Treatment that I may experience. I acknowledge all the above and agree to these Terms & Conditions for any future Treatments I have done by Lit Smiles Perth.
